ABSTRACT

Background: Thyroid gland is an endocrine organ. The non-neoplastic and neoplastic conditions affecting the gland can manifest as the swelling of the gland with thyroid dysfunction. Based on cytology, the diseases can be categorized into non-neoplastic, benign neoplastic, and malignant neoplastic conditions and according to hormone status, the lesions of thyroid can be categorized in hypothyroid, euthyroid, or hyperthyroid condition. Aims and Objectives: Our study was done to find out the various cytomorphological spectrum of thyroid diseases and to record their common clinical presentation along with hormonal status in a tertiary care hospital in Northern Odisha. Materials and Methods: A total of 220 cases of thyroid disorder were taken for analysis over a period of 2 years. Patients demographic data, brief clinical features, finding on FNAC (cytomorphology), and thyroid function status were analyzed with appropriate statistical method. Results: The predominant age group affected was 21–40 years and total female to male patient ratio was 5.5:1. Most common presentations were heat intolerance (22.27%), cold intolerance (28.63%), and tachycardia (27.72%) apart from thyroid enlargement. Most common non-neoplastic lesion were multinodular and colloid goiter constituting 38.18% of total cases and most common neoplastic lesion was papillary carcinoma constituting 14.55% of total cases. Thyroid function test showed predominantly euthyroid states for all conditions. Conclusion: As the rest part of India, this part of Odisha also showed that thyroid disorders are more common in females and the most affected age group is 21–40 years. Multinodular and colloid goiter are the most common thyroid lesions. Most of the cases present with euthyroid state.

ABSTRACT

Lateral flow immunoassay (LFIA) is a rapid detection technique that allows researchers to move the antigen-antibody reaction from a test tube or laboratory vessel to a test strip. Due to the chromatographic effect of the test strip, the solution would move to a specified direction based on the test and complete the whole antigen-antibody specific reaction. A qualitative judgment can be made with the naked eye by observing the color change of the reagent strip at a specific location. Because of its advantages of being fast, simple, specific, inexpensive, and requiring no specialized personnel, LFIA is now widely used in medical testing, food quality monitoring, environmental monitoring, agriculture and animal husbandry. A major bottleneck for the development of LFIA technology is the hook effect. This paper summarizes the current methods, means and research progresses to combat the hook effect, hoping to provide a strong technical reference for researchers to design test strips, select suitable nanoparticles, and achieve quantitative LFIA detection.

ABSTRACT

Objective:To evaluate the clinical value of NG-Test Carba5 for rapid detection of carbapenemases produced by carbapenem-resistant Enterobacteriaceae (CRE) strains. Methods:A total of 1 210 CRE strains were collected during 2018-2022 from 77 hospitals in 21 provinces of China and were subjected to NG-Test Carba5 for rapid detection of carbapenemase. The whole genome sequencing (WGS) analysis was referenced as the gold standard method.Results:Overall, the NG-Test Carba5 demonstrated excellent performance in detection of five kinds of carbapenemases [Klebsiella pneumoniae carbapenemase (KPC), New Delhi metallo-β-lactamase (NDM), imipenemase metallo-β-lactamase (IMP), Verona integron-encoded metallo-beta-lactamase (VIM) and oxacillinase-48-type carbapenemases(OXA-48)] from CRE strains, with a sensitivity of 98.47% (1 161/1 179), specificity of 100% (31/31), and positive predictive value of 100% (1 161/1 161). The sensitivity for detection of NDM, IMP, OXA and VIM reached 100% (307/307), and 97.70% (763/781) for KPC. For 11 strains carrying blaKPC-25, blaKPC-78, or blaKPC-93, NG-Test Carba5 reported positive KPC detection (11/11). For strains carrying blaKPC-33 and blaKPC-77, however, NG-Test Carba5 delivered negative results. Additionally, for those strains co-producing two or three kinds of carbapenemases, NG-Test Carba5 was able to report all of the targets with a sensitivity of 100% (91/91). Conclusions:NG-Test Carba5 showed excellent performance in rapid and accurate detection of carbapenemases from CRE strains. Nonetheless, for those strains with negative results, some other phenotypic and genotypic methods should be implemented alongside to avoid missing targets.

Resumen El milium coloide (MC) es un trastorno de depósito cutáneo poco común, asociado a cambios degenerativos secundarios a la radiación ultravioleta, que provoca degeneración de las fibras elásticas en la dermis. Tiene dos formas de presentación definidas como juvenil y del adulto. Es más común en hombres que realizan oficios al aire libre. Clínicamente se caracteriza por numerosas y pequeñas pápulas de distintos colores (amarillo, ámbar, café) o translúcidas, que suelen agruparse y se localizan en zonas fotoexpuestas. El diagnóstico se confirma mediante el estudio histopatológico y pudiera requerir tinciones especiales para diferenciar la mucina y el amiloide. Para su tratamiento se han empleado técnicas como dermoabrasión mecánica, láseres, terapia fotodinámica, entre otros.


Abstract Colloid milium is a rare skin deposition disorder associated with degenerative changes secondary to ultraviolet radiation, which causes degeneration of elastic fibers in the dermis. It has two forms of presentation defined as juvenile and adult. It is more common in men who work outdoors. Clinically it is characterized by numerous small yellow, brown, amber or translucent papules that are usually grouped together, located over photo-exposed areas. The diagnosis is confirmed by histopathological study and may require special stains to differentiate mucin and amyloid. Techniques such as mechanical dermabrasion, lasers, photodynamic therapy, among others, have been used for its treatment.

ABSTRACT

Objetivo : Determinar el rendimiento diagnóstico de la técnica de inmnunocromatografía o flujo lateral para la detección de anticuerpos en pacientes con Fasciolosis humana. Materiales y métodos : Estudio observacional, prospectivo y de corte transversal. Hemos desarrollado una prueba de flujo lateral (Fasciorap) para el diagnóstico serológico de las Fasciolosis humana por Fasciola hepatica, compuesta por antígenos de excreción-secreción de formas adultas conjugadas con orocoloidal de 40 nm y una proteína A e IgG de conejo anti Fasciola hepatica como reactivos detectores en la línea de prueba y control, flanqueados por almohadillas en un cassette. Se evaluaron 240 sueros, 120 positivos, 50 sueros de pacientes con otras parasitosis, 20 de pacientes con enfermedades infecciosas y 50 sueros de personas no parasitadas, la interpretación de resultados se realizó por inspección visual a los 15 minutos de aplicada las muestras. Resultados : La prueba detectó la presencia de anticuerpos en el suero de pacientes con fasciolosis, alcanzando una sensibilidad de 92,5%, una especificidad de 94,17%, un valor predictivo positivo de 94,07% y negativo de 92,62%; con 100% de concordancia en la repetibilidad y reproducibilidad. Conclusiones : Fasciorap detecta casos de fasciolosis, por lo tanto, es una potencial prueba diagnóstica en zonas endémicas donde se requiere pruebas de punto de atención


Objective : To determine the diagnostic performance of the immunochromatography technique or lateral flow for the detection of antibodies in patients with human fasciolosis. Materials and methods : Observational, prospective and cross-sectional study. We have developed a lateral flow test (Fasciorap) for the serological diagnosis of fasciolosis due to F. hepatica, composed of excretion-secretion antigens of adult forms conjugated with orocolloid of 40 nm and a protein A and IgG of rabbit anti F. hepatica as detector reagents in the test and control line, flanked by pads in a cassette. 240 sera were evaluated, 120 positive, 50 sera from patients with other parasites, 20 from patients with infectious diseases and 50 sera from non-parasitized people, the interpretation of results was performed by visual inspection 15 minutes after applying the samples. Results : The test detected the presence of antibodies in the serum of patients with fasciolosis, reaching a sensitivity of 92.5%, a specificity of 94.17%, a positive predictive value of 94.07% and a negative predictive value of 92.62%; with 100% agreement on repeatability and reproducibility. Conclusions : Fasciorap detects cases of fascioliasis, therefore, it is a potential diagnostic test in endemic areas where point-of-care testing is required

ABSTRACT

Objective:To explore the significance of 99Tc m-sulfur colloid lymphoscintigraphy in the diagnosis of lower limb lymphedema after gynecological tumor surgery. Methods:The clinical data of patients with lower limb lymphedema after gynecological tumor surgery in Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology from May 2015 to October 2019 were retrospectively analyzed. 99Tc m-sulfur colloid lymphoscintigraphy was performed in all patients. The results of lymphatic vessel imaging, lymph node imaging and their combination in the diagnosis of lower limb lymphedema were analyzed. The diagnostic efficacy of lymphatic vessel imaging alone, lymph node imaging alone and their combination was evaluated by the receiver operating characteristic (ROC) curve and the area under the curve (AUC), and the Youden index, sensitivity and specificity were calculated. Results:Among the 100 lower limbs of 50 patients, 56 limbs had lymphedema and 44 limbs had no obvious edema. When diagnosis was based on abnormal lymphatic vessel imaging alone, among 56 lower limbs with lymphedema, lower limbs lymphatic vessel imaging was positive in 38 (67.9%) and negative in 18 (32.1%); among 44 lower limbs without obvious edema, lower limbs lymphatic vessel imaging was positive in 6 (13.6%) and negative in 38 (86.4%); the sensitivity was 67.9%, the specificity was 86.4%, and the Youden index was 0.543. When diagnosis was based on abnormal lymph node imaging alone, among 56 lower limbs with lymphedema, lower limbs lymph node imaging was positive in 42 (75.0%) and negative in 14 (25.0%); among 44 lower limbs without obvious edema, lower limbs lymph node imaging was positive in 13 (29.5%) and negative in 31 (70.5%); the sensitivity was 75.0%, the specificity was 70.5%, and the Youden index was 0.455. When diagnosis was based on the combination of lymphatic vessel imaging and lymph node imaging, among 56 lower limbs with lymphedema, lymphatic vessel imaging and lymph node imaging were positive in 48 (85.7%) and negative in 8 (14.3%); among 44 lower limbs without obvious edema, lymphatic vessel imaging and lymph node imaging were positive in 14 (31.8%) and negative in 30 (68.2%); the sensitivity was 85.7%, the specificity was 68.2%, and the Youden index was 0.539. The AUC for the combined diagnosis of lymphatic vessel imaging and lymph node imaging was 0.781, the AUC for the diagnosis of abnormal lymphatic vessel imaging was 0.771, and the AUC for the diagnosis of abnormal lymph node imaging was 0.739 (all P < 0.01). Conclusions:99Tc m-sulfur colloid lymphoscintigraphy is of great help in the diagnosis of lower limb lymphedema after operation of gynecological tumors. The combination of lymph node imaging and lymphatic vessel imaging is more effective in the diagnosis of lower limb lymphedema.

Introducción: La ascitis se define como la presencia de líquido en la cavidad peritoneal. La etiología más común la constituyen las enfermedades hepáticas que cursan con hipertensión portal, dentro de ellas la cirrosis hepática reportada en un 40 por ciento, a 5 años de seguimiento de los casos. Estudios previos muestran que la ascitis por enfermedad cirrótica hepática es entre un 80-85 por ciento, también la carcinomatosis se presenta en un 10 por ciento, y dentro de las causas más raras están la insuficiencia cardiaca y la tuberculosis peritoneal en un 3 por ciento, junto con la trombosis de la vena porta, sarcoidosis, tumores intraperitoneales, ascitis pancreática y la enteritis eosinofílica. Objetivo: Describir presentaciones clínicas infrecuentes como causa de ascitis en pacientes hospitalizados en el Hospital Clínico Quirúrgico Hermanos Ameijeiras. Presentación de casos: Se presentan cinco casos que ingresaron en el Hospital Hermanos Ameijeiras con diagnóstico de Ascitis: quilosa, hemorrágica, eosinofílica y coloide, con las patologías que los llevaron a esa manifestación clínica y una breve descripción de la misma. Conclusión: El análisis de las presentaciones clínicas de los casos, los hallazgos en los estudios imagenológicos y en los exámenes de laboratorio, así como los resultados anatomopatológicos permitieron el diagnóstico de las entidades causantes de ascitis atípicas en estos pacientes(AU)


Introduction: Ascites is defined as the presence of fluid in the peritoneal cavity. The most common etiology is liver diseases with portal hypertension; among them liver cirrhosis is reported in 40 percent of cases with 5-year follow-up. Previous studies demonstrate that ascites due to cirrhotic liver disease occurs in 80-85 percent of the cases, that carcinomatosis is also present in 10 percent, and also that among the rarest causes, heart failure and peritoneal tuberculosis are present in 3 percent of cases along with portal vein thrombosis, sarcoidosis, intraperitoneal tumors, pancreatic ascites and eosinophilic enteritis. Objective: To describe uncommon clinical presentations as cause of ascites in patients admitted to Hermanos Ameijeiras Clinical Surgical Hospital. Case presentation: Five cases of patients admitted to Hermanos Ameijeiras Clinical Surgical Hospital with the diagnosis of ascites: chylous, hemorrhagic, eosinophilic and colloid as well as the pathologies that led them to this clinical manifestation and a brief description of it are presented. Conclusion: The analysis of the clinical presentation of the cases, the findings in the imaging studies and laboratory tests and the anatomopathological results allowed the diagnosis of the entities causing atypical ascites in these patients(AU)

ABSTRACT

Objective:To investigate the imaging features of colloid carcinoma arising from intraductal papillary mucinous neoplasm (IPMN) of pancreas and the differentiation features from ductal adenocarcinoma arising from IPMN, using the pathological findings as the reference.Methods:Twenty-four patients with pathologically confirmed colloid carcinoma from November 2013 to January 2020 in Changhai Hospital, Navy Medical University were included in this study. The clinical manifestations, imaging features and pathological data were retrospective reviewed. Thirty patients of ductal adenocarcinoma arising from IPMN confirmed by pathology were selected as the control group. CT and MRI features of two groups were blindly analyzed by two radiologists, including the lesions location, type of IPMN, size, components, density or signal, calcification, dilation and size of the main pancreatic duct (MPD), pancreatic parenchymal atrophy, fistula formation. The χ 2 test or Fisher exact probability was used to compare the imaging features between the two groups. Results:As for IPMN with colloid carcinoma, 16 cases were located in the head of the pancreas, 7 cases in the body and tail of the pancreas, and 1 case showed diffused changes of the pancreas. Mass was found in twenty-two cases, with the size of 54.5 (29) mm. Nineteen cases were solid-cystic, 4 were cystic and 1 was solid. Thick wall and internal separation with mild enhancement were displayed. Five cases were found with high signal on T 1WI. Thirteen cases had calcification and 2 cases had gas in the tumor. The size of MPD was (13±5) mm. Pancreatic parenchymal atrophy was found in 21 cases and fistula formation was found in 8 cases. The mass size of IPMN with colloid carcinoma was significantly greater than that of IPMN with ductal adenocarcinoma [31 (16) mm, Z=-3.758, P<0.001]. Solid-cystic mass was more found in IPMN with colloid carcinoma and solid mass was more found in IPMN with ductal adenocarcinoma ( P<0.001). Calcification ( P=0.001), fistula formation ( P=0.031), and high signal on T 1WI ( P=0.034) were more found in IPMN with colloid carcinoma than IPMN with ductal adenocarcinoma. Conclusion:Compared with IPMN with ductal adenocarcinoma, the solid-cystic mass, calcification, fistula formation and high signal on T 1WI were more commonly found in IPMN with colloid carcinoma.

Background: Diseases of the thyroid gland are one of the most common endocrine disorders in India and the world, many of these diseases require surgery for their treatment. The incidence of thyroid diseases differs from one geographical area to another. This study was aimed at finding out the thyroid pathologies for which thyroid surgeries were performed in D. Y. Patil Hospital, Nerul, Navi Mumbai.Methods: It is a retrospective analysis of all the patients who have undergone thyroid surgeries (lobectomy, hemithyroidectomy, subtotal or near total thyroidectomy or total thyroidectomy) in D. Y. Patil hospital, Nerul, Navi Mumbai from 1st January 2018 to 31st December 2019 (2 years duration).Results: Most common pathology for which thyroidectomy was performed was colloid goitre (41.33%). Benign diseases of the thyroid was much more common (76%) than malignant diseases (24%). Most common malignancy was papillary carcinoma thyroid. Thyroid surgeries were most commonly performed on women in their 5th decade of life.Conclusions: Colloid goitre was the most common pathology for which thyroid surgeries were performed and females in their 5th decade of life were the most common patients undergoing thyroidectomy.

ABSTRACT

Colloid cysts are benign, thin walled spherical neoplasms, composed of a collagenous capsule, underlying epithelium that arises from brain’s epidermal embryonic remnants and a viscous centre. They are shown to approximate 0.5% of all intracranial tumours with no recorded evidence of the petrous temporal bone involvement. Colloid cyst of the petrous temporal bone often presents with clinical symptoms of headache, hearing loss, facial palsy and imbalance/vertigo. Which is diagnosed on radiological and histological findings. Histologically, they are lined by ciliated cuboidal to pseudo stratified columnar epithelium resting on an eosinophilic basement membrane. Imaging Techniques are helpful in early diagnosis and preventing further complications. Here we will be discussing about a 24-year-old female, a known case of petrous apex osseous haemangioma presenting with unstable gait and tingling sensation on one side of face leading to an incidental finding of a colloid cyst on petrous temporal bone through histological examination.

Background: Thyroid nodules are a common entity encountered in clinical practice and the prevalence by palpation is only 3-7%, but on ultrasonographic evaluation it is 20-76% with wide variability. Accurate prediction of malignancy with minimal diagnostic modalities and assessing the risk factors in malignancy may help in reducing extensive procedures. The objective of the study is to assess the risk factors associated with development of malignancy in solitary thyroid nodules and to find out the percentage of malignancy among the cases of solitary nodules.Methods: A three year prospective study was conducted at ACSR medical college and all cases diagnosed as solitary thyroid nodules were enrolled and institutional ethical approval was obtained. Socio demographic data, clinical history and ultrasonogram findings were collected and entered in a separate data sheet for analysis. Fine needle aspiration cytology, histopathological examination was done and reported findings were noted. Statistical analysis was done in SPPS version 20 and ‘p’ value <0.05 was considered significant.Results: 128 cases of solitary thyroid nodules (STN) with 41 male and 87 female cases and F:M ratio of 2.13:1 with mean age of 46.6±10.8 years was enrolled. 43.75% were euthyroid, 47.66% were hypothyroid and 8.59% were hyperthyroid on hormonal evaluation. Micro calcification was observed in 50.78%, increased vascularity in 38.28% and irregular margins in 34.38% of cases. Colloid goitre was predominant observation on fine needle aspiration cytology (40.6%) and follicular adenoma (32%) on histopathology.Conclusions: Features of micro calcification, solid echogenicity, and associated lymphadenopathy are associated features with increased risk of malignancy among STN.

Background: Annual incidence of thyroid nodules ranges from 40,000 to 70,000 per 1 lac population worldwide. Fine Needle Aspiration (FNA) of the thyroid is widely accepted simple, cost effective and quick to perform outpatient procedure with minimal complication. The National Cancer Institute (NCI) Bethesda, Maryland, United States standardized the reporting system for thyroid FNA by using Bethesda system for reporting thyroid cytopathology.Methods: FNA was performed in total 155 patients presenting with the thyroid swelling with or without Ultrasonography (USG) guidance. All patients were analyzed for age, gender, type of lesions. FNA was done and smears were examined and reported according to Bethesda system of reporting for thyroid cytopathology. Findings were correlated with post-operative histopathological diagnoses in 103 cases who underwent surgery.Results: Out of total 155 patients, 32 were male and 123 were female. Average age of presentation was 38.4 years. On cytology, according to Bethesda system, most cases were in benign category (76.1%) followed by malignant (8.4%). On histopathological study most common diagnosis was colloid goiter followed by papillary thyroid carcinoma. From the study it was found that sensitivity, specificity and accuracy of fine needle aspiration cytology of thyroid lesions were 81.8%, 97.3% and 95.4% respectively.Conclusions: Fine needle aspiration cytology is simple, easy to perform, cost effective procedure with high sensitivity, specificity and diagnostic accuracy in case of thyroid lesions.

ABSTRACT

Plant polyphenols have a wide range of pharmacological activities and application prospects. Liquid polyphenol preparations have special physical phases and complex chemical compositions, with problems such as poor stability and easy precipitation during production and marketing. Taking the multi-precipitation mechanism of plant polyphenol liquid preparations as an example,we discuss the chemistry and composition of the precipitation, how it forms, whether precipitationcan be controlled, and the interaction law of three precipitation approaches. An unstable mechanism model is proposed where hydrolyzed tannin hydrolysis and catechin non-enzymatic oxidative polymerization repeatedly induces associative colloid aggregation and precipitation. This study explains the complex physicochemical changes in polyphenol solutions and the microcosmic mechanism of instability in the induced system and proposes a steady state reconstruction of liquid polyphenol preparation consistent with the common law of precipitation and control. It has scientific significance for promoting the development and manufacture of high quality liquid polyphenol preparations.

INTRODUCTION@#Intracranial colloid cysts are rare benign tumors located in the region around the foramen of Monro or around the third ventricle with an annual incidence of 3 of 1,000,000. The common clinical picture is a progressive headache due to the rapid enlargement of the cyst, resulting in hydrocephalus as it obstructs the cerebrospinal flow; brain herniation may ensue, leading to death. @*OBJECTIVES@#The objectives of this report are to (1) present a case of intracranial colloid cyst in a young female with neurofibromatosis type 1, (2) emphasize the importance of early diagnosis by clinical signs and symptoms and (3) highlight the importance of neuroimaging in arriving at a neurologic diagnosis.@*CASE REPORT@#A nineteen-year-old female was seen due to a three-month history of progressive headache, with associated signs of increased intracranial pressure and with a medical history of neurofibromatosis type 1, inherited from her mother. Neurologic findings revealed papilledema and 6th cranial nerve palsy. By radiographic imaging with clinical correlation, patient was diagnosed with a colloid cyst obstructing the foramen of Monro. Surgical intervention was done and patient improved without complications. @*DISCUSSION@#Neurofibromatosis is a hereditary neurocutaneous syndrome in which the skin, nervous system, bones, endocrine glands and sometimes other organs are the sites of a variety of congenital abnormalities, often taking the form of benign tumors. Intracranial colloid cyst was seen in this case report. Colloid cysts have an incidence of 0.5 – 1% of all primary brain tumors and are the most common masses in the third ventricle and in the Foramen of Monro. They cause obstruction of CSF flow, resulting in hydrocephalus. No published case of neurofibromatosis type 1 patients with a symptomatic intracranial colloid cyst has been reported yet. Surgical resection is the treatment of choice for colloid cyst.

Objective@#To establish a colloidal gold technique assay for the rapid detection of immunoglobulin(Ig) M and IgG antibodies against 2019 novel coronavirus (2019-nCoV) and to evaluate its clinical performance.@*Methods@#A total of 278 patients who were treated at Wuhan Hankou Hospital and the People's Hospital of Honghu from February 12, 2020 to February 20, 2020 were collected. According to the diagnostic criteria, 89 patients were confirmed with 2019-nCoV nucleic acid positive diagnosis, and 189 were 2019-nCoV nucleic acid-negative suspected patients. A total of 273 medical examiners from Nanfang Hospital, Southern Medical University from 2015 to 2018 were selected as controls. The serum samples of patients were collected. 2019-nCoV nucleic proteins were obtained from prokaryotic expression vectors. Indirect IgM and IgG colloidal gold techniques were established by using recombinant N protein. 2019-nCoV nucleic acid detection by reverse transcription-polymerase chain reaction (RT-PCR) was used as control. Serum specimens were tested for 2019-nCoV IgM and IgG. The specificity and sensitivity of colloidal gold assay were analyzed.@*Results@#The sensitivity and specificity of IgM detection reagents were 78.7% and 98.2%, respectively, those of IgG detection reagents were 73.0% and 99.3%, respectively, and those of IgM combined with IgG detection were 87.6% and 98.2%, respectively. For suspected patients with negative 2019-nCoV nucleic acid, the positive rates of IgM and IgG were 59.8% (113/189) and 52.9% (100/189), respectively, and the positive rate of IgM combined with IgG detection was 66.1% (125/189).@*Conclusion@#This reagent of 2019-nCoV antibodies detection (colloidal gold technique) fulfills the requirement for clinical application with high specificity and sensitivity, which can be served as a supplementary detection method for 2019-nCoV nucleic acid detection by RT-PCR.

ABSTRACT

Introduction: Colloid cysts are one of the benign intracranial tumors most commonly occurring in the rostral part of the thirdventricle. These may present with varied spectrum of clinical features that poses challenges in clinical diagnosis. The presentationmay range from being asymptomatic to simple headaches, seizures, and even sudden death. Most of the symptoms can beattributed to the development of obstructive hydrocephalus. Chemical or aseptic meningitis is unusual complication posingcomplicating differential diagnosis. We describe eight such cases with wide variety of symptoms.Materials and Methods: We present a case series of eight cases of the third ventricle colloid cysts presented at our institute.Age of the patients ranged from 15 to 55 and five of them were females. All the clinical features were recorded from eachone of them. Computed tomography and magnetic resonance imaging were used to diagnose the condition. Four of themunderwent excision of the cyst in single stage either by open or endoscopic approach. Two patients underwent preliminaryventriculoperitoneal shunt done in the view of poor neurological status and craniotomy and excision was done in later stage. Inone patient bedside external ventricular drain was inserted for emergency decompression of ventricles. One patient is underserial radiological follow-up.Results: Eight cases that we observed had wide variety of symptoms. Six patients had chronic headache with progressiveseverity, and four of them had nausea with vomiting, three patients had seizures. The cysts in two patients were discoveredaccidentally, during the evaluation of seizures in one patient and others in evaluation of traumatic head injury. One elderly patienthad presented with psychiatric symptoms, drop attacks along with the features of normal pressure hydrocephalus. One teenagepatient presented with sudden deterioration and went into cardiac arrest even after emergency decompression of ventriclesdone. Seven of them underwent surgery and one of them succumbed. The surgery improved health in all other seven patients.Conclusion: Colloid cysts may present with a wide range and beyond expected neurological manifestations. The severity orrapid clinical deterioration does not exactly correlate with depending on the site, size of the cyst. Leaking cysts with chemicalmeningitis may further complicate the diagnosis. Hence, early diagnosis and surgery with complete removal of cysts offer betterclinical outcomes in those patients.

ABSTRACT

Ellagic acid is ubiquitous in plants and is considered as a potential candidate for antioxidant and antineoplastic drugs. However, ellagic acid has poor solubility and precipitates easily even after initial solubilization. Improvement of its bioavailability has been a concern of pharmaceutical industry. It was found that storage in Sanlejiang oral liquid at low temperature keeps its stability. Ellagic acid is anomalous in a way that is easily soluble at low temperatures but precipitates at high temperatures. In order to reveal the mechanism of this phenomenon and develop precipitation prevention and control strategies, ellagic acid in Sanlejiang oral liquid was stored at high, medium and low temperatures for three months. The changes of composition and phase state of the whole system during storage were systematically tracked and studied by means of precipitation amount or morphology, HPLC chemical profile of supernatant versus precipitates, and comprehensive characterization of physical phase state. The results show that the amount of precipitation at low temperature is only 1/3 of that at normal room temperature. As the temperature rises, the sedimentation increases sharply. HPLC analyses of supernatant versus precipitates revealed that ellagic acid precipitation originated from two ways: chemical degradation and physical deposition. The chemical sedimentation is related to the hydrolysis of tannins under acidic condition, forming chebulagic acid and corilagin. Physical sedimentation is related to the decrease of the association degree and viscosity of polyphenol colloids when temperature rises. This study elucidated the stability mechanism of ellagic acid in liquid preparations of TCM, and provided the mechanistic basis for efficient utilization and solution prepartion of ellagic acid.

ABSTRACT

ObjectiveTo investigate the clinical effect of infusion of albumin versus artificial colloidal fluid after ascites drainage in patients with liver cirrhosis and ascites. MethodsCochrane Library (from 1993 to February 2018) PubMed (from 1966 to February 2018), Embase (from 1990 to February 2018), Chinese Scientific Journal Full-Text Database (from 1994 to February 2018), CBM (from 1978 to February 2018), China Science and Technology Journal Database (from 1989 to February 2018), Chinese Medical Association Digital Periodical Database (from 1997 to February 2018), and related periodicals and conference proceedings were searched for randomized controlled trials (RCTs) on infusion of albumin and artificial colloidal fluid after ascites drainage in patients with liver cirrhosis and ascites. The modified JADAD method and Cochrane systematic review were used for data extraction and literature quality assessment, and a statistical analysis was performed. RevMan 53 was used for the Meta-analysis. ResultsA total of 7 RCTs with 696 patients were included, with 299 patients in the human serum albumin group and 397 in the artificial colloidal fluid group. The human serum albumin group had a significantly lower incidence rate of hyponatremia than the artificial colloidal fluid group (11.04% vs 20.4%, risk ratio [RR]=0.58, 95% confidence interval [CI]: 0.40-0.84, P=0.004). There were no significant differences between the two groups in the incidence rates of kidney injury (702% vs 7.81%, RR=0.93, 95%CI: 0.53-1.65, P=0.82), hepatic encephalopathy (6.77% vs 7.45%, RR=0.87, 95%CI: 0.48-1.55, P=0.63), gastrointestinal bleeding (3.91% vs 3.65%, RR=0.97, 95%CI: 0.43-2.22, P=0.95), abdominal infection (522% vs 4.56%, RR=1.07, 95%CI: 052-2.18, P=0.86), and hospital death (12.78% vs 14.59%, RR=0.70, 95%CI: 047-1.02, P=0.06). ConclusionHuman albumin has an advantage over artificial colloidal fluid in reducing hyponatremia after ascites drainage in patients with cirrhotic ascites.

ABSTRACT

@#Objective    To investigate the effect of artificial colloid on coagulation function in pediatric patients weighing less than 5 kg with congenital heart disease during cardiopulmonary bypass in congenital heart disease surgery by using artificial colloid instead of human serum albumin. Methods    A total of 65 pediatric patients with weight less than 5 kg who underwent congenital heart disease surgery in our hospital from September 2016 to December 2017 were included in the study. They were randomly divided into two groups: an artificial colloid group (the experimental group, n=33) and a human serum albumin combined artificial colloid group (the control group, n=32). Perioperative hemoglobin concentration (Hb), blood products and hemostatic drugs used, postoperative coagulation function index and pleural fluid volume 24 hours after surgery were monitored. Results    There was no significant difference in perioperative Hb and chest tube drainage between the two groups. The platelet utilization rate in the experimental group was significantly lower than that in the control group (P<0.05). No significant difference was found in the dosage of other blood products and hemostatic drugs between the two groups during the perioperative period. There was no significant difference in coagulation parameters between the two groups before and after surgery (P>0.05). Conclusion    The use of artificial colloid as colloid priming solution during cardiopulmonary bypass has no adverse effect on coagulation function in pediatric patients weighting less than 5 kg with congenital heart disease.

ABSTRACT

Background: Diseases of thyroid are one of the most common endocrine disorders affecting general population which range from non-neoplastic to neoplastic lesions. The incidence and pattern of thyroid lesions depend on various factors which include sex, age, ethnic and geographical patterns. Majority of thyroid lesions are non-neoplastic only <5% are malignant. The aim of the present study was to determine the frequency and histomorphological pattern of thyroidectomy specimens and their relationship with age and sex of the patient.Methods: This retrospective study was conducted in the department of pathology, Govt. Medical College, Alappuzha for a period of 2 years. The study included 620 thyroidectomy specimens received in the Department of Pathology. All the biopsy reports were reviewed, and different lesions were categorised according to age and gender distribution. The data was analysed by standard statistical methods.Results: The commonest of the non-neoplastic lesions was nodular colloid goiter followed by lymphocytic thyroiditis, Hashimoto thyroiditis Nodular hyperplasia and thyroglossal cyst. Most common malignant lesion in this study is papillary carcinoma and benign lesion is follicular adenoma. Age group of patients ranged from 6 ½ to 84 years. The study showed a female predominance of 88.38%.Conclusions: Thyroid disorders are commonly encountered endocrine diseases. The study showed a female predominance. Peak age of incidence of thyroid lesions was between 40 and 50 years. Most common lesion was follicular adenoma and most common malignant lesion was papillary carcinoma.Histopathological examination is the mainstay for definite diagnosis and management of thyroid neoplasms.
